    Preschool 2024-2025 Pre-Registration Form

    We offer preschool using a sliding fee scale. If you request tuition fee assistance, income verification documents will be required to complete this pre-registration process. You will need one of documents listed below available to upload before completing this form. You cannot save progress once you start the form.
  • Preschool Tuition: All classes are 2 hour and 45 minutes per day

    2 days per week $156 per month 3 and 4 year old classes
    3 days per week $234 per month 3 and 4 year old classes
    4 days per week $311 per month 4 year old classes
    5 days per week (Voluntary Prekindergarten) Free for specific attendance areas only

    One time annual $49 registration fee due with first monthly payment (9 payments total)

  • If you are a resident of Anoka-Hennepin School District and you live in or attend child care in the Adams, Evergreen, Monroe, University or Madison attendance area, your four-year-old is eligible for Voluntary Prekindergarten. If you are unsure, please use the following link and enter your address or your child care address to determine your attendance area.

  • The next question asks for your pre-tax income.

    Income includes:

    • Income from employment
    • Child Support
    • Alimony payments
    • Other Cash Assistance from state or local government
    • Unemployment benefits
    • Worker’s compensation
    • Veteran’s benefits
    • Strike benefits
    • Social Security, disability benefits
    • Regular income from trusts or estates
    • Annuities, Investment income, Rental income
    • Regular cash payments from outside household
  • You will be asked to provide your income verification documents below. Submit the most current documentation available for all income sources.

    If you are not receiving fee assistance and will pay Full Tuition, income verification documentation is not required.

    Acceptable proof includes: 

    • The previous year’s Income Tax Return (preferred)
    • Previous year W-2 form
    • Public Assistance: Letter from agency providing benefits which states amount of benefits/checks
    • Financial aid statement, or a statement from an employer on company letterhead
    • Letter verifying child is legal responsibility of the court (Foster child)
    • Pension, SSI, Retirement Social Security: Letter from agency providing benefits which states amount of benefits/checks
    • Unemployment, Worker’s Compensation, strike benefits: Copies of 3 most recent checks
    • Most recent (consecutive) 30 days of pay stubs for each income earner
    • Other Income (i.e. farm, self-employment, etc.): previous year Form 1040 and Schedule C
