Dear Parent/Guardian,

Please complete this form if your child(ren) live outside their assigned school's attendance area or outside of the Anoka-Hennepin district boundary, but wish to make a request to utilize bussing services.  Students not living inside their school's assigned attendance area may only ride to and from existing bus stops that are within the attendance area.  Routes and stops will not be created outside this area.

If your student(s) will attend a day care facility, Adventures Plus site, or other alternate residence inside the school's attendance area, where they would need transportation services, please complete the "Day Care/Alternate Address Transportation Request Form".

If you need to make a change in this request during the school year, please complete and submit a new form.  It could take 3-5 business days for the change to take effect, and during this time it is the parent/guardian's responsibility to transport.

For new school year priority, please complete this form before August 1st.

By signing this form, the parent/guardian(s) acknowledge responsibility for safely presenting their student(s) at their assigned bus stop as well as having plans in place for when their student(s) are dropped off again after school.
