Dear Parent/Guardian,
Unless informed otherwise, the Anoka-Hennepin Schools Transportation Service Department will assume all eligible bus riders will be picked up and dropped off at the stop nearest their primary home address. Please complete this form if your child(ren) are split between parents/guardians who no longer live together in the same household. Both parents/guardians must have joint legal/physical custody and both must have primary addresses located inside their child(ren)'s school attendance area. Please note, students will be scheduled on bus routes for both locations and it is the parent/guardian's responsibility to inform school staff and your child(ren) which bus they should be riding home each day.
If one, or both, of the parent/guardian's primary addresses are located outside of their child(ren)'s attendance area, please make sure to complete the "Out-of-Attendance Area Transportation Request Form" instead of this one. If your student(s) will also attend a day care facility, Adventures Plus site, or other alternate residence where they would need transportation services, please complete the "Day Care/Alternate Address Transportation Request Form".
If you need to make a change in either household addresses during the school year, please complete and submit a new form. It could take 3-5 business days for the change to take effect, and during this time it is the parent/guardian's responsibility to transport.
For new school year priority, please complete this form before August 1st. Only one parent/guardian is required to submit this form, if authorized, on behalf of both parents/guardians.
